INJURIES / RETURN TO PLAY
CONCUSSION PROTOCOL
The health and safety of our players is of the utmost importance and we are committed to doing our part to ensure a fun and safe experience for all. With that, hockey is a physical sport that can result in injuries and concussions. In the event that a player sustains a potential concussion, VOSHA follows the USA Hockey practice of "when in doubt, sit them out."
In order to ensure the safety of our players, the following return to play plan has been put in place for players and families.
- Remove the player from activity immediately.
- The Team Manager or VOSHA Staff must complete the USA Hockey Incident Report.
- The Team Manager must email the concussion packet and AAHA Return to Play Form to the parents of the player.
- Parents must email the completed AAHA Return to Play Form to the Titans Administrator before the player can return to the ice.
Player may return to play after they have been cleared by a physician and all forms have completed and submitted. Players may not return to play until after all forms have been submitted to VOSHA.
STEP 1
Reporting
The Team Manager or VOSHA coaching staff member must complete the USA Hockey injury reporting form the day the incident occurs and email the completed form to the VOSHA Administrator and AAHA Representative.
STEP 2
PARENT CONTACT
The Team Manager must email concussion forms and resources to the parents of the player and notify them that their child cannot return to practice, games, etc until the form is completed and they are cleared by an approved physician.
STEP 3
RETURN TO PLAY
Parents must email the completed return to play form to he Titans Admin once the child is cleared by a doctor. Once confirmed and cleared, the family, Team Manager, and Coaches will be notified that the player is cleared to play.

Injury Reporting
In the event of an injury during a Titans team event, a VOSHA representative is required to complete documentation. Depending on the nature of the injury, the player may be asked to seek medical attention and/or be limited from practices and games until it is deemed safe for them to return. For injuries that are sustained outside of team activities, we ask that parents and/or the player inform their coaching staff as well as the Team Manager.
